Choosing the Right Penile Enlargement Surgery in Turkey for Length or Girth

Penile enlargement is not one operation but a group of methods that add length, girth or both, often in combination. Assoc. Prof. Dr. Tuncay Taş, a urologist in Istanbul, sets out what each method realistically adds, what it costs in recovery and risk, and which patients it suits, so the choice is made on evidence rather than on promises.

Penile Enlargement at a Glance

MethodAddsRealistic gainLasts
Ligament releaseLength2 to 4 cm flaccid, 1 to 2 cm erectPermanent, with traction
Fat transferGirth1.5 to 3 cmPermanent after 30 to 50% absorption

Be wary of any single procedure that promises both length and girth in one session. Good results usually come from a personalised combination of methods, chosen after examination.

Average Penis Size

Visible length depends on three structures: the erectile bodies (corpora cavernosa), the suspensory ligament that anchors the penis to the pubic bone and keeps part of it hidden, and the fat pad above the pubic bone, which can conceal 1 to 3 cm.

The Veale meta-analysis in BJU International, covering more than 15,500 men, gives these averages:

MeasurementAverageNormal range
Flaccid length9.16 cm6.5 to 11.8 cm
Erect length13.12 cm10.7 to 15.5 cm
Erect girth11.66 cm9.7 to 13.6 cm

In adults, micropenis means an erect length below 9.3 cm. Anything above that is medically normal, and treatment is a personal choice rather than a medical need.

Estimate Your Result

Enter four simple measurements to see an estimate of your length and girth after surgery. It gives a first idea only; the real assessment is made at examination. The tool also has its own page, the penis enlargement calculator.

Step 1 of 4

What is your erect penis length?

With a full erection, measure the distance from the pubic bone at the base of your abdomen to the tip of the penis.

How to measure

  • Measure while fully erect.
  • Lay the ruler along the top of the penis and press it gently into the pubic bone.
  • Read the distance from the bone to the tip (where the head ends).
  • If you carry extra weight, pressing the fat pad down makes the reading more accurate.

Expectations and Penile Dysmorphia

In clinical practice, 80 to 85 percent of men asking for enlargement are of normal size. When the perceived size is very different from the real one, this is called penile dysmorphic disorder, and the first step is sex therapy and, where needed, psychological support rather than surgery or injections.

Warning signs include:

  • Checking size many times a day
  • Avoiding changing rooms, beaches or saunas
  • Avoiding sex or changing partners because of size
  • Comparing yourself with pornography
  • Several previous enlargement procedures

A psychosocial assessment before surgery is an ethical requirement, especially for men unhappy with an earlier procedure.

Length or Girth: Choosing the Goal

Most men want both, but deciding which matters more shapes the whole plan.

Which Method Suits Whom

Wants a Reversible Result

Hyaluronic acid filler, which can be dissolved. A sensible first step.

Wants the Biggest Change at Once

The Spartacus package: ligament release, liposuction and fat transfer together.

Permanent Girth

Fat transfer, or semi permanent PMMA filler.

Length Without Surgery

Traction device, weight control and vacuum therapy.

Erection Problems and Size

A modern penile implant restores rigidity and adds some volume.

Buried Penis

Suprapubic liposuction with ligament release uncovers the hidden shaft.

Ligament Release for Length

The suspensory ligament anchors the penis to the pubic bone. Releasing it lets part of the shaft that sits inside the body come forward.

  • General or spinal anaesthesia, 60 to 90 minutes
  • A 2 to 3 cm incision hidden in the pubic hair
  • One night in hospital
  • Gain: 2 to 4 cm flaccid, 1 to 2 cm erect
  • A traction device for six months; without it 30 to 50 percent of the gain can be lost

The trade offs are a 5 to 15 degree drop in the erect angle, four to six weeks without sex, and a limited effect on erect length.

Fat Transfer for Girth

Fat is taken from the abdomen or inner thigh by mini liposuction, purified and injected under the skin of the shaft.

  • Local anaesthesia with light sedation, 60 to 90 minutes
  • 30 to 60 ml transferred; home the same day
  • Your own tissue, so no allergy and a natural feel
  • 30 to 50 percent absorbed in the first six months
  • One or two top up sessions are often needed
  • Uneven absorption, cysts or nodules in 5 to 15 percent

Dermal Grafts

A graft of the patient's own skin tissue is placed under the skin of the shaft. Unlike fat it is barely absorbed, so the girth gain is more durable. The cost is a second surgical site, a longer recovery and a technique that fewer centres offer.

Suprapubic Liposuction

Fat above the pubic bone can hide 1 to 3 cm of the base of the penis, a condition called hidden or buried penis. Removing it increases visible length by 1 to 2 cm without touching the penis itself or affecting erections.

  • Local anaesthesia, no visible scar
  • Compression garment for three to four weeks
  • Weight gain can reverse the effect

The Spartacus Package

The Spartacus package combines several procedures in one session, aiming for the largest visible change in both length and girth from a single recovery.

  1. Suspensory ligament release, for length
  2. Suprapubic liposuction, to uncover the base
  3. Fat transfer or hyaluronic acid filler, for girth
  4. Glans enlargement, optional
  5. Traction device for the following six months
  6. Pelvic floor exercise and lifestyle plan

Expected result: 3 to 4 cm of flaccid length, 1.5 to 2.5 cm of erect length and 2 to 3 cm of girth. It needs general anaesthesia and four to six weeks of recovery.

The package does not suit everyone. Age, anatomy, other conditions and a psychosocial assessment decide it, and satisfaction is highest in men who tried non surgical options first.

Penile Implant When Erections Are Weak

When erectile dysfunction and a feeling of small size come together, a three piece inflatable implant solves both in one operation: it restores reliable rigidity and modern devices add some volume. Satisfaction is above 95 percent. See penile implant surgery in Turkey for the details.

Neither adds length. Both avoid stitches and scars, and most men are back at work the next day.

Non Surgical Options

  • Traction device: worn 4 to 8 hours a day for 6 to 12 months, adds 1 to 2 cm of erect length. It is also used after ligament release to keep the gain.
  • Vacuum device: temporary swelling with a small lasting gain; mainly a rehabilitation tool.
  • PRP, exosome and stem cell treatments: aimed at tissue quality and firmer erections, with only an indirect effect on size.
  • Shockwave therapy: improves blood flow and erection quality, which can increase perceived size.
  • Pelvic floor exercises: better erection quality and control, no direct effect on size.

What the Evidence Says

Enlargement surgery is not standard urological treatment for a normal sized penis. The American Urological Association's policy on penile augmentation does not regard suspensory ligament division or fat injection as proven safe and effective, and long term controlled studies remain few.

That is why the gains on this page are given as ranges, why reversible and non surgical options are offered first, and why men with penile dysmorphic disorder are referred for support before any procedure. The most reliable predictors of a good outcome are realistic expectations and an experienced urologist.

Risks and Complications

  • Common and mild: swelling, bruising, tenderness, temporary change in sensation, asymmetry after filler or fat
  • Moderate: filler lumps, fat cysts or nodules in 5 to 15 percent, a 5 to 15 degree drop in erect angle after ligament release, infection in 1 to 2 percent after surgery
  • Serious and rare: granuloma after PMMA, vein damage and erectile problems from poorly injected fat, permanent loss of sensation

Silicone oil and other unapproved substances injected into the penis can destroy tissue and should never be used.

Most complications depend on who performs the procedure. Choose a urologist working in a licensed hospital with approved materials.

Every figure in the Turkey column is a typical package price including the procedure, anaesthesia and hospital care where needed, and follow up. Prices for other countries are typical private, self pay ranges. Enlargement is cosmetic, so insurance does not cover it. Flights and hotel are not included. Your exact price depends on the plan agreed at assessment, and you can request a personal quote through the contact page.

Planning Treatment From Abroad

  1. Send your measurements, photographs if you wish, and medical history for review before booking
  2. Day 1: consultation, measurement and blood tests in Istanbul
  3. Day 2: the procedure
  4. Wound check before you fly: after one or two days for filler, four to five days for fat transfer or ligament release, five to seven days for the Spartacus package
  5. Video follow up after you return home, with traction guidance where needed
