Years after a vasectomy, Vasectomy Reversal in Turkey can bring sperm back for natural conception

A vasectomy is meant to be permanent, but it can be undone. Microsurgery rejoins the cut ends of the vas deferens, and within ten years of the vasectomy sperm returns in around nine in ten men. In Istanbul, the same surgeon performs both vasovasostomy and vasoepididymostomy, choosing between them during the operation.

What Is a Vasectomy Reversal?

A vasectomy cuts and seals the vas deferens, the tubes that carry sperm from the testicles. The testicles keep making sperm, but it has nowhere to go. A reversal reconnects the tubes, making natural conception possible again without IVF.

It does not change testosterone, sex drive or erections. Other causes of infertility are checked separately, as in male infertility assessment.

Who Is a Vasectomy Reversal For?

  • A new partner who would like children
  • Family plans that have changed since the vasectomy
  • Wishing for another child after the loss of a child
  • Persistent scrotal pain after vasectomy

Before surgery, the original vasectomy is reviewed, the scrotum is examined and hormone tests are done where needed. The partner's age and fertility are assessed too.

Vasovasostomy and Vasoepididymostomy

The technique is chosen during surgery, after fluid from the testicular side of the vas is examined under the microscope. Each side is judged on its own.

Vasovasostomy

When the fluid contains sperm, the scarred ends are trimmed and rejoined with sutures finer than a hair. It takes two to three hours for both sides, sperm usually returns within two to three months, and it has the highest success rate.

Vasovasostomy stitches the two cut ends back into one tube.

Vasoepididymostomy

Years of back pressure can cause a second blockage in the epididymis. If the fluid contains no sperm, the vas is joined to the epididymis above it. It takes three to four hours, sperm can take six to twelve months to appear, and it is more often needed ten or more years after the vasectomy.

Many reversals fail because only the simpler vasovasostomy is offered. The surgeon should be able to perform both techniques, so a second blockage is never left untreated.

Vasectomy Reversal Success Rates

Time since the vasectomy is the strongest predictor. Figures from the Vasectomy Study Group, the largest published series:

Years since vasectomySperm back in the semenPregnancy
Under 3 yearsAbout 97 percentAbout 76 percent
3 to 8 yearsAbout 88 percentAbout 53 percent
9 to 14 yearsAbout 79 percentAbout 44 percent
15 years or moreAbout 71 percentAbout 30 percent

Pregnancy also depends on the partner's age and fertility. Results after a failed reversal are usually lower.

Reversal or Sperm Retrieval With IVF?

Sperm can instead be taken from the epididymis or testicle (PESA, MESA or TESE) for IVF with ICSI.

Vasectomy reversalSperm retrieval + ICSI
ConceptionNatural, at homeIVF with ICSI
More childrenNo further treatmentA new IVF cycle each time
Treatment for partnerNoneEgg stimulation and collection
Often preferred whenPartner is younger, more children plannedPartner is 40 or over, or needs IVF anyway

Retrieval also suits men whose reversal has failed. For retrieval techniques, see Micro-TESE surgery.

How the Operation Is Done

  1. Under general or spinal anaesthesia, the vas ends are exposed through small scrotal cuts.
  2. Fluid from the testicular end is checked for sperm.
  3. Sperm present: the ends are rejoined (vasovasostomy).
  4. No sperm: the vas is joined to the epididymis (vasoepididymostomy).
  5. The other side is assessed the same way.
  6. Sperm found can be frozen as a back-up.

Recovery Timeline

WhenWhat to expect
Day of surgeryHome the same day or after one night
First few daysAching eased with painkillers and scrotal support
Before flyingWound check after a few days of rest

About 3 weeksSex, gym and heavy lifting
6 to 8 weeksFirst semen analysis, which can be done near home
Every 2 to 3 monthsRepeat semen analysis until counts settle or pregnancy

Risks of Vasectomy Reversal

  • Bruising and swelling, which are expected
  • Haematoma or wound infection, which are uncommon
  • Sperm not returning to the semen
  • Late narrowing of the join

If the join narrows, sperm frozen during surgery can be used for ICSI without another operation.
